- Be referred to another healthcare provider when services you need are beyond the scope of those offered by Circle of Hope.
Questions and Concerns

Your Responsibilities
To help us provide safe, effective, and compassionate care, we ask that you:
- Provide complete and accurate information about your health, medical history, medications, and symptoms.
- Inform us of changes to your health or contact information.
- Ask questions whenever something is unclear.
- Follow agreed-upon treatment and follow-up recommendations or let us know if you choose not to do so.
- Keep scheduled appointments whenever possible or notify us if you need to cancel or reschedule.
- Treat our staff, volunteers, other clients, and facilities with courtesy and respect.
- Respect the privacy of other clients receiving services.
- Follow safety policies while visiting our center.
