At Circle of Hope, we are committed to protecting the privacy of your protected health information (“PHI”). PHI includes information that identifies you and relates to your past, present, or future physical or mental health, the healthcare you receive, or payment for that care.

• Provide you with this Notice of Privacy Practices.
• Follow the terms of the Notice currently in effect.
• Notify you if a breach of your unsecured protected health information occurs, as required by law.

How We May Use and Disclose Your Health Information

For Treatment

We may use and share your health information to provide, coordinate, or manage your healthcare. For example, healthcare providers involved in your care may review ultrasound reports, laboratory results, or other medical information to help provide appropriate treatment.

For Healthcare Operations

We may use your information to improve the quality of care we provide, conduct quality assurance activities, train staff, maintain records, comply with licensing requirements, and manage our healthcare operations.

As Required by Law

We may disclose health information when required by federal, state, or local law.

Public Health and Safety

Questions or Complaints

If you believe your privacy rights have been violated, you may file a complaint with Circle of Hope or with the U.S. Department of Health and Human Services, Office for Civil Rights.

We will not retaliate against you for filing a complaint.
